These prices are effective January 1, 2012,
and are subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. (However, any funeral arrangements you select will include a charge for our basic services and overhead.) If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
PROFESSIONAL SERVICES
| BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF | $2,095.00 |
Our charge includes, but is not limited to, consultation with the family or responsible party; staff and facilities to respond to initial requests for service; coordination of service plans with cemetery, crematory and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations. Also included in this charge are overhead expenses related to our facilities such as professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds maintenance, taxes, furnishings and secretarial and administrative expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.)

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

| ALTERNATIVE SANITARY CARE AND HOLDING UP TO 24 HOURS | $245.00 |
This fee is for topical disinfection and holding of an unembalmed body for up to 24 hours prior to cremation or burial. (An unembalmed body may present health hazards.)

Our facility does not have refrigeration and would contract with outside providers on a cost per day basis, plus transportion charges to and from the location of refrigeration.

Our charge for a direct cremation (without viewing or ceremony) includes: Initial transfer of the deceased from place of death, basic services of funeral director and staff, transportation to crematory and return of the cremated remains to the authorized representative. This charge does not include the cremation fee or Ohio permit fee. These fees average around $250.00. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like cardboard, fiberboard or composition materials (with or without an outside covering). The containers we provide are constructed of cardboard.

SERVICE PACKAGES
A list detailing services included in each package will be provided at the funeral home. NO Funeral Goods (casket, outer burial container, urn, etc.) are included in the package prices shown below. These packages reflect a discount from our itemized service prices. The discount will vary based on the services and funeral goods selected. While additions may be made to packages, no credit will be given for items not used in conjunction with the package selected. Every package includes one Memorial Candle and Online Guest Book.

The only warranties, expressed or implied, granted in connection with the goods sold with this funeral service are the express written warranties, if any, extended by the manufacturers thereof. No other warranties, expressed or implied, including the implied warranties of merchantability or fitness for a particular purpose, are extended by the funeral home.
PAYMENT POLICY
| FUNERAL GOODS AND SERVICES |
Payment is due no later than 30 days from the date of service. A service charge of 1% per month (12% annually), or the rate allowed by law, whichever is lower, will be due and payable on past due accounts.

3) We accept assignment of Insurance Policies. Please provide us with the insurance policy prior to services and we will verify that the policy is in force and an assignment can be made.

Payment is due on or before date of service. We will advance payment on your behalf for items such as, newspapers, death certificates, honorariums, sales tax, grave openings, etc. We request payment for these items by the day of the service in the form of Cash, Check or Credit Card.
